Summary
Beta-blockers can modestly raise potassium by impairing cellular potassium uptake, and together with irbesartan this may add to hyperkalemia risk. Both lower blood pressure, so additive hypotension and bradycardia-related symptoms can occur. This combination is often used intentionally.
What this interaction means in plain words
Irbesartan and Nadolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Irbesartan belongs to Cardiovascular; Nadolol belongs to Cardiovascular.
Why it happens
Additive antihypertensive effects, and reduced renin release and cellular potassium uptake from nonselective beta-blockade combined with reduced renal potassium excretion from the ARB.
What to do
Monitor blood pressure and heart rate, and check potassium periodically, especially with renal impairment.
