Summary
Volatile anesthetics such as isoflurane can prolong the QT interval, and loperamide at high exposure has been associated with QT prolongation and torsades de pointes. Concurrent use could add to arrhythmia risk, especially if loperamide has been taken in excess or the patient has other risk factors. At normal therapeutic loperamide doses the added risk is modest.
What this interaction means in plain words
Isoflurane and Loperamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Isoflurane belongs to Central Nervous System; Loperamide belongs to Gastrointestinal Tract.
Why it happens
Additive QT prolongation through cardiac ion channel effects (hERG blockade by loperamide at high concentrations, repolarization effects of isoflurane).
What to do
Ask about loperamide use before anesthesia and avoid high-dose use perioperatively. Monitor ECG and correct potassium and magnesium if other QT risk factors are present.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
