Summary
Both drugs can prolong the QT interval, so combined use may increase the risk of torsades de pointes. Lofexidine also causes bradycardia and hypotension, which add to the arrhythmic risk.
What this interaction means in plain words
Itraconazole and Lofexidine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Itraconazole belongs to Anti Fungals; Lofexidine belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic QT prolongation; itraconazole may also modestly raise lofexidine exposure through CYP inhibition.
What to do
Avoid the combination when possible. If it is used, obtain a baseline ECG, correct potassium and magnesium, and monitor QTc, heart rate and blood pressure.
What to ask your pharmacist
