Summary
Itraconazole increases lorlatinib exposure, which may worsen CNS effects, hyperlipidemia, and PR interval prolongation. Lorlatinib is itself a CYP3A inducer and may lower itraconazole concentrations, risking loss of antifungal efficacy.
What this interaction means in plain words
Itraconazole and Lorlatinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Itraconazole belongs to Anti Fungals; Lorlatinib belongs to Oncology.
Why it happens
Itraconazole strongly inhibits CYP3A4, which metabolizes lorlatinib, while lorlatinib induces CYP3A and may reduce itraconazole levels.
What to do
Avoid concurrent use and consider a non-CYP3A-inhibiting antifungal. If unavoidable, reduce the lorlatinib dose per labeling, monitor ECG and lipids, and check antifungal response or itraconazole levels.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
