Summary
Pantoprazole raises gastric pH, which markedly reduces the absorption of itraconazole capsules and may cause subtherapeutic antifungal levels and treatment failure. The effect is much less with the oral solution or the acidic cola co-administration approach.
What this interaction means in plain words
Itraconazole and Pantoprazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Itraconazole belongs to Anti Fungals; Pantoprazole belongs to Gastrointestinal Tract.
Why it happens
Higher gastric pH decreases dissolution and absorption of itraconazole capsules, which need an acidic environment.
What to do
Use the oral solution or an SUBA-itraconazole formulation, which are less pH-dependent, or stop the acid suppressant if possible. Monitor itraconazole trough levels and clinical response.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
