Summary
Primidone is metabolized to phenobarbital, an enzyme inducer, which can substantially lower itraconazole concentrations. This may lead to antifungal treatment failure.
What this interaction means in plain words
Itraconazole and Primidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Itraconazole belongs to Anti Fungals; Primidone belongs to Central Nervous System.
Why it happens
Induction of CYP3A4 by primidone and phenobarbital increases itraconazole clearance.
What to do
Avoid the combination if possible and use an alternative antifungal or anticonvulsant. If it cannot be avoided, measure itraconazole levels and adjust the dose.
