Summary

Saquinavir, usually given with ritonavir, inhibits CYP3A4 and may modestly increase lacosamide exposure. Saquinavir can also prolong PR and QT intervals, so combining it with lacosamide raises conduction concerns.

What this interaction means in plain words

Lacosamide and Saquinavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

CYP3A4 inhibition may raise lacosamide levels, with additive effects on cardiac conduction.

What to do

Check an ECG before starting in patients with cardiac disease or other conduction-affecting drugs, and monitor for bradycardia, dizziness and syncope. Lower the lacosamide dose if tolerability is poor.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
