Summary

Fluoroquinolones can cause hypoglycemia or hyperglycemia in patients on insulin secretagogues. Repaglinide stimulates insulin release, so the risk of hypoglycemia may be raised.

What this interaction means in plain words

Levofloxacin and Repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Levofloxacin belongs to Anti Bacterial; Repaglinide belongs to Endocrine.

Evidence level: Moderate

Why it happens

Fluoroquinolones alter beta-cell insulin release, which may add to the secretagogue effect of repaglinide.

What to do

Monitor blood glucose closely during the antibiotic course and teach the patient about hypoglycemia. Reduce the repaglinide dose if low glucose occurs.

What to ask your pharmacist
