Summary
Fluoroquinolones such as levofloxacin can cause both low and high blood glucose in patients on glucose-lowering drugs. Sitagliptin alone rarely causes hypoglycemia, so the risk is mainly unpredictable dysglycemia in diabetic patients, especially older adults or those with renal impairment.
What this interaction means in plain words
Levofloxacin and Sitagliptin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Levofloxacin belongs to Anti Bacterial; Sitagliptin belongs to Endocrine.
Why it happens
Fluoroquinolones alter pancreatic beta-cell insulin release, producing dysglycemia that may add to the glucose-lowering effect of sitagliptin.
What to do
Advise the patient to monitor blood glucose more often during the levofloxacin course and to report symptoms of hypoglycemia or hyperglycemia. Stop levofloxacin and switch antibiotic class if significant glucose abnormalities occur.
