Summary

Levofloxacin can prolong the QT interval, and beta-2 agonists such as vilanterol may add to this effect through hypokalemia and adrenergic stimulation. Together they may increase the risk of ventricular arrhythmias including torsades de pointes.

What this interaction means in plain words

Levofloxacin and Umeclidinium-vilanterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Levofloxacin belongs to Anti Bacterial; Umeclidinium-vilanterol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive QT prolongation with beta-agonist-related hypokalemia and sympathetic stimulation.

What to do

Check potassium and magnesium and correct abnormalities. Consider ECG monitoring in patients with other QT risk factors, or choose a non-QT-prolonging antibiotic.
