Summary

Both drugs lower serum potassium, so together they can cause clinically significant hypokalemia. This raises the risk of ventricular arrhythmias, particularly with high-dose or repeated levosalbutamol use, in hypoxic patients, or with other arrhythmia risk factors.

What this interaction means in plain words

Levosalbutamol and Metolazone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Levosalbutamol belongs to Respiratory; Metolazone belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive hypokalemia, from beta-2-mediated intracellular potassium shift with levosalbutamol and renal potassium loss with metolazone.

What to do

Monitor serum potassium and magnesium, especially during acute asthma or COPD exacerbations or when levosalbutamol use is high. Replace potassium as needed.

What to ask your pharmacist
