Metolazone added to a regimen containing hydrochlorothiazide produces sequential nephron blockade with a markedly greater diuretic effect. This can cause profound hypokalemia, hyponatremia, volume depletion and hypotension, especially in older patients or those with heart failure or renal impairment.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Metolazone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Metolazone belongs to Cardiovascular.
Why it happens
Both agents inhibit distal tubular sodium reabsorption, producing additive natriuresis and potassium and magnesium loss.
What to do
Avoid routine combination of two thiazide-type diuretics; if intentional, monitor electrolytes, renal function, weight and blood pressure closely after starting. Consider reducing doses and correcting potassium and magnesium promptly.
