Summary

Tranylcypromine is an irreversible non-selective MAO inhibitor, and linezolid adds further MAO inhibition. This stacks the risks of severe serotonin syndrome and hypertensive crisis.

What this interaction means in plain words

Linezolid and Tranylcypromine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Linezolid belongs to Anti Bacterial; Tranylcypromine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive inhibition of MAO-A and MAO-B causing excess serotonin and catecholamines.

What to do

Do not combine them; use a non-MAO-inhibiting antibiotic. In an urgent case where linezolid is essential, stop tranylcypromine and observe closely, recognising the prolonged MAO inhibition after discontinuation.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
