Summary
Metolazone can reduce renal lithium clearance and raise serum lithium concentrations. This can lead to lithium toxicity with tremor, confusion, ataxia, gastrointestinal upset and, in severe cases, seizures or renal injury.
What this interaction means in plain words
Lithium and Metolazone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lithium belongs to Central Nervous System; Metolazone belongs to Cardiovascular.
Why it happens
Sodium depletion from the diuretic increases proximal tubular reabsorption of lithium, lowering its renal clearance.
What to do
Avoid the combination where possible; if used, reduce the lithium dose and check serum lithium levels within the first week and after any diuretic dose change. Counsel the patient to recognise early toxicity signs and maintain hydration and sodium intake.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
