Summary
Tricyclic antidepressants such as lofepramine can change the cardiovascular response to amphetamines. Methamphetamine's indirect effects may be reduced, but its pressor and arrhythmogenic effects may also be potentiated, and both drugs can lower seizure threshold. The net result is unpredictable and can include hypertension, tachycardia and arrhythmia.
What this interaction means in plain words
Lofepramine and Methamphetamine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Lofepramine blocks norepinephrine reuptake and the neuronal uptake of indirect sympathomimetics, producing unpredictable pharmacodynamic changes in adrenergic response.
What to do
Avoid the combination where possible and choose a non-sympathomimetic alternative. If used together, monitor blood pressure, heart rate and ECG and adjust doses based on response.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
