Summary

Both agents can prolong QTc, and ranolazine inhibits CYP2D6, which may raise tricyclic antidepressant levels. Together this increases the risk of arrhythmia and anticholinergic or cardiac toxicity.

What this interaction means in plain words

Lofepramine and Ranolazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

Additive QTc prolongation plus ranolazine inhibition of CYP2D6 metabolism of lofepramine's active metabolite.

What to do

Avoid if possible; otherwise monitor ECG and electrolytes and use the lowest effective lofepramine dose.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
