Summary
Both drugs can prolong the QT interval and cause sedation and hypotension. Risperidone can also inhibit CYP2D6 weakly, and tricyclics such as lofepramine are partly metabolized by this enzyme, so combined exposure and adverse effects may increase.
What this interaction means in plain words
Lofepramine and Risperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive QT prolongation and CNS depression, with possible minor CYP2D6-related pharmacokinetic effects.
What to do
Obtain a baseline ECG and monitor it, and correct potassium and magnesium abnormalities. Monitor for sedation and orthostatic hypotension and reduce doses if needed.
What to ask your pharmacist
