Summary

Lofexidine is a central alpha-2 agonist that lowers heart rate and blood pressure, and metoprolol has similar effects through beta-blockade. Used together they can produce marked bradycardia, hypotension and syncope. Lofexidine also prolongs the QT interval, which adds to the concern in susceptible patients.

What this interaction means in plain words

Lofexidine and Metoprolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Lofexidine belongs to Central Nervous System; Metoprolol belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic reduction of heart rate and blood pressure through central sympatholytic and beta-adrenergic blocking actions.

What to do

Monitor heart rate and blood pressure closely, especially during lofexidine initiation and dose titration, and reduce or hold doses for symptomatic bradycardia or hypotension. Consider an ECG in patients with other QT risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
