Summary

Lomitapide is a sensitive CYP3A4 substrate and moderate CYP3A4 inhibitors such as aprepitant can substantially raise its levels. This increases the risk of hepatotoxicity and gastrointestinal adverse effects.

What this interaction means in plain words

Aprepitant and Lomitapide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Aprepitant belongs to Gastrointestinal Tract; Lomitapide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated lomitapide metabolism by aprepitant.

What to do

Avoid per lomitapide labeling, which contraindicates moderate and strong CYP3A4 inhibitors; use an antiemetic without CYP3A4 inhibition, such as ondansetron.
