Summary
Lonafarnib is a sensitive CYP3A substrate, and its exposure rises when it is given with CYP3A inhibitors. Prednisolone is not a clinically important CYP3A inhibitor, so any rise in lonafarnib levels is probably small. Higher lonafarnib exposure could increase gastrointestinal toxicity and QT-related arrhythmia risk.
What this interaction means in plain words
Lonafarnib and Prednisolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Lonafarnib belongs to Miscellaneous; Prednisolone belongs to Endocrine.
Why it happens
Possible weak inhibition of CYP3A4-mediated lonafarnib metabolism, which is predicted rather than well established for prednisolone.
What to do
If the combination is needed, continue lonafarnib at the current dose and monitor for palpitations, syncope and ECG changes. Check electrolytes and consider a baseline and follow-up ECG.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
