Summary

Lorcaserin is a serotonin 5-HT2C agonist that inhibits CYP2D6 weakly, which may raise risperidone exposure. Combining serotonergic and dopamine-blocking agents also raises theoretical concern for serotonin syndrome or neuroleptic malignant syndrome. Lorcaserin has been withdrawn from the US market.

What this interaction means in plain words

Lorcaserin and Risperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

CYP2D6 inhibition by lorcaserin may increase risperidone levels, plus possible additive serotonergic and antidopaminergic effects.

What to do

Monitor for extrapyramidal symptoms, sedation and signs of serotonin toxicity such as agitation, tremor, hyperthermia and clonus. Reduce the risperidone dose if adverse effects emerge.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
