Summary
NSAIDs such as lornoxicam reduce renal prostaglandin synthesis, which can impair potassium excretion and renal function. With spironolactone this raises the risk of hyperkalemia and acute kidney injury, and the NSAID can reduce spironolactone's diuretic and antihypertensive effect.
What this interaction means in plain words
Lornoxicam and Spironolactone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Lornoxicam belongs to Musculoskeletal; Spironolactone belongs to Cardiovascular.
Why it happens
Aldosterone antagonism combined with NSAID-induced reduction in renal prostaglandins, suppressed renin and reduced renal perfusion, decreasing potassium excretion.
What to do
Avoid the combination where possible, or use the lowest NSAID dose for the shortest time. Monitor serum potassium and creatinine within the first week, particularly in patients with renal impairment or heart failure.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
