Tolmetin may reduce the antihypertensive effect of losartan. Together they can also cause deterioration of renal function and hyperkalemia, particularly in older or volume-depleted patients.
What this interaction means in plain words
Losartan and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Losartan belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins combined with ARB-mediated efferent dilation reduces glomerular filtration and potassium excretion.
What to do
Monitor blood pressure, creatinine and potassium, particularly in the first weeks of combined therapy. Ensure hydration and avoid in patients with significant renal impairment.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
