Summary
Lumacaftor is a strong CYP3A inducer and may lower modafinil concentrations, reducing efficacy. Modafinil is also partly CYP2C19-dependent, and lumacaftor/ivacaftor have mixed inducing and inhibiting effects on CYP2C19, so net changes are unpredictable.
What this interaction means in plain words
Lumacaftor / ivacaftor and Modafinil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Lumacaftor induction of CYP3A4 (and variable CYP2C19 effects) alters modafinil clearance.
What to do
Monitor for reduced wakefulness response to modafinil and titrate the dose to effect.
