Summary
Mifepristone, particularly at the higher doses used for Cushing syndrome, prolongs the QT interval, and maprotiline can do the same. Combined use may increase the risk of serious ventricular arrhythmia. Mifepristone also inhibits CYP2D6, which may raise maprotiline concentrations.
What this interaction means in plain words
Maprotiline and Mifepristone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Maprotiline belongs to Central Nervous System; Mifepristone belongs to Endocrine.
Why it happens
Additive QT prolongation, plus CYP2D6 inhibition by mifepristone that may increase maprotiline exposure.
What to do
Use an alternative antidepressant if possible. If the combination is needed, obtain a baseline ECG, correct electrolytes, monitor the QTc, and consider reducing the maprotiline dose.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
