Summary

NSAIDs can blunt the blood pressure lowering effect of ACE inhibitors. In volume-depleted, elderly or renally impaired patients the combination can also precipitate acute kidney injury and hyperkalemia.

What this interaction means in plain words

Mefenamic acid and Trandolapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Mefenamic acid belongs to Musculoskeletal; Trandolapril belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAID inhibition of vasodilatory renal prostaglandins opposes ACE inhibitor effects and reduces glomerular perfusion.

What to do

Use the lowest effective NSAID dose for the shortest time, maintain hydration, and monitor blood pressure, creatinine and potassium. Prefer paracetamol in high-risk patients.
