Summary

NSAIDs can reduce the diuretic and natriuretic effect of loop diuretics and may cause fluid retention. In susceptible patients such as those with heart failure, cirrhosis or kidney disease, this can lead to worsening edema and acute kidney injury.

What this interaction means in plain words

Meloxicam and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Meloxicam belongs to Musculoskeletal; Torsemide belongs to Cardiovascular.

Evidence level: High

Why it happens

NSAIDs inhibit renal prostaglandin synthesis, reducing renal blood flow and blunting loop diuretic-induced natriuresis.

What to do

Monitor weight, edema, blood pressure and renal function, and increase the torsemide dose if diuretic response falls. Use the lowest NSAID dose for the shortest time, particularly in volume-depleted patients.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
