Summary

Procarbazine has MAO-inhibiting properties, and meperidine with MAO inhibitors can cause severe, sometimes fatal serotonin syndrome or excitatory reactions with hyperthermia. Meperidine is the opioid most associated with this reaction.

What this interaction means in plain words

Meperidine and Procarbazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Meperidine belongs to Musculoskeletal; Procarbazine belongs to Oncology.

Evidence level: Moderate

Why it happens

MAO inhibition combined with meperidine's serotonin reuptake inhibition leading to serotonin excess.

What to do

Do not use together or within 14 days of stopping an MAO inhibitor. Choose a non-serotonergic analgesic such as morphine, with monitoring.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
