Summary
Protriptyline can potentiate the cardiovascular effects of amphetamines by blocking norepinephrine reuptake, and may also reduce the indirect effects by blocking amphetamine uptake into adrenergic neurons. Hypertension, tachycardia and arrhythmias may occur, and both drugs can lower seizure threshold. Protriptyline is also a CYP2D6 substrate, and amphetamines may raise tricyclic levels.
What this interaction means in plain words
Methamphetamine and Protriptyline can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Protriptyline blocks norepinephrine reuptake and neuronal uptake of methamphetamine, altering adrenergic response unpredictably.
What to do
Avoid the combination if an alternative exists. If needed, monitor blood pressure, heart rate and ECG, and use the lowest effective doses.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
