Summary

Combining amphetamines with an MAO inhibitor can cause a massive release of norepinephrine and dopamine that cannot be metabolized. This can cause hypertensive crisis, hyperthermia, arrhythmia and serotonin toxicity.

What this interaction means in plain words

Methamphetamine and Selegiline transdermal are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

MAO inhibition blocks amine breakdown while methamphetamine promotes release of catecholamines and serotonin.

What to do

Do not co-administer, and separate by at least 14 days after stopping an MAO inhibitor before starting methamphetamine. Use a non-amphetamine option, and treat any hypertensive or serotonergic emergency immediately.

What to ask your pharmacist
