Summary

Both drugs enhance adrenergic activity, so combined use can produce marked rises in blood pressure and heart rate, along with anxiety, tremor and agitation. Patients with cardiovascular disease are at particular risk of hypertensive episodes or arrhythmias.

What this interaction means in plain words

Methamphetamine and Yohimbine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

Additive sympathomimetic effects: methamphetamine releases norepinephrine and yohimbine blocks presynaptic alpha-2 receptors, increasing norepinephrine release.

What to do

Avoid combining where possible, especially in patients with hypertension or heart disease. If it cannot be avoided, monitor blood pressure and heart rate and watch for agitation or palpitations.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
