Summary

Methylergonovine is a strong ergot vasoconstrictor, and rizatriptan adds to this effect. Prolonged vasospasm with ischemia of the heart, brain or limbs may occur.

What this interaction means in plain words

Methylergonovine and Rizatriptan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Additive vasoconstriction through 5-HT1B/1D agonism and ergot adrenergic and serotonergic effects.

What to do

Avoid use together and separate by at least 24 hours. Use a non-ergot uterotonic such as oxytocin when bleeding must be controlled.
