Summary

Ticarcillin is given as a sodium salt with a high sodium load and can lower serum potassium, which may add to the potassium wasting caused by metolazone. Hypokalemia can become significant in patients on high-dose ticarcillin. Both drugs are also renally cleared.

What this interaction means in plain words

Metolazone and Ticarcillin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive potassium loss through diuretic-induced kaliuresis and ticarcillin's nonreabsorbable anion and sodium load increasing distal potassium excretion.

What to do

Monitor serum potassium and magnesium regularly and replace as needed. Adjust ticarcillin dose for renal function.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
