Summary

Metyrapone inhibits cortisol synthesis and its effect is judged by cortisol and ACTH response. Exogenous prednisone suppresses the pituitary-adrenal axis and cross-reacts in some cortisol assays, so it can invalidate the metyrapone test and blunt its effect.

What this interaction means in plain words

Metyrapone and Prednisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Metyrapone belongs to Endocrine; Prednisone belongs to Endocrine.

Evidence level: Moderate

Why it happens

Exogenous glucocorticoid suppresses ACTH feedback and counteracts cortisol lowering, interfering with metyrapone testing and therapy.

What to do

Avoid giving prednisone around a metyrapone stimulation test unless required. If used for adrenal insufficiency during therapy, use a regimen guided by an endocrinologist.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
