Summary
Voclosporin is a CYP3A4 substrate with a narrow therapeutic range. Mifepristone inhibition of CYP3A4 can raise voclosporin concentrations, increasing the risk of nephrotoxicity, hypertension and neurotoxicity.
What this interaction means in plain words
Mifepristone and Voclosporin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Mifepristone belongs to Endocrine; Voclosporin belongs to Miscellaneous.
Why it happens
Mifepristone inhibits CYP3A4 metabolism of voclosporin.
What to do
Avoid concurrent use where possible. If necessary, reduce the voclosporin dose per labeling and monitor eGFR and blood pressure.
