Mirogabalin + Pregabalin

For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Overview

Severity: Moderate · Evidence level: Moderate

Therapeutic duplication of gabapentinoids with additive CNS depression (sedation, dizziness, ataxia), cognitive impairment, and potential respiratory depression; increased risk of peripheral edema. Limited clinical rationale for concurrent use.

Mechanism of action

Additive pharmacodynamic effect as alpha-2-delta calcium channel ligands; minimal pharmacokinetic interaction expected since both are renally eliminated with negligible CYP involvement.

Clinical management

Avoid concurrent use when possible; select a single gabapentinoid. If cross-tapering, use the lowest effective doses with brief overlap and close monitoring for CNS depression and respiratory status. Adjust doses for renal function and counsel on fall risk and impaired alertness.
