Summary

Perphenazine is a sedating phenothiazine, so combining it with morphine increases CNS depression, hypotension and respiratory depression risk. Perphenazine inhibits CYP2D6, but morphine depends little on that pathway, so the pharmacodynamic effect dominates.

What this interaction means in plain words

Morphine and Perphenazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Morphine belongs to Musculoskeletal; Perphenazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive CNS depression and hypotension; CYP2D6 inhibition is of minor relevance to morphine.

What to do

Use the lowest effective doses and monitor sedation, blood pressure and respiratory rate, especially in the elderly.

What to ask your pharmacist

  • Should I take them at different times of day?
