Summary
NSAIDs reduce renal prostaglandin synthesis, which can blunt the antihypertensive effect of sacubitril-valsartan and impair renal function, especially in elderly, volume-depleted or renally impaired patients. Hyperkalemia risk may also rise.
What this interaction means in plain words
Nabumetone and Sacubitril-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Nabumetone belongs to Musculoskeletal; Sacubitril-valsartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of vasodilatory prostaglandins reduces renal perfusion and sodium excretion, adding to the effects of angiotensin receptor blockade on glomerular filtration and potassium handling.
What to do
Avoid regular NSAID use where possible. If needed, use the lowest dose for the shortest time and monitor renal function, serum potassium and blood pressure.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
