Summary
NSAIDs can weaken the antihypertensive effect of angiotensin receptor blockers. In older, volume-depleted or renally impaired patients the combination may cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Nabumetone and Valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Nabumetone belongs to Musculoskeletal; Valsartan belongs to Cardiovascular.
Why it happens
NSAID inhibition of vasodilatory renal prostaglandins reduces glomerular perfusion, which is critical when angiotensin II action is blocked, and promotes sodium retention and potassium retention.
What to do
Monitor blood pressure, serum creatinine and potassium after starting or changing the NSAID, and keep the patient well hydrated. Avoid regular NSAID use in high-risk patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
