Summary
Aminoglycosides can strengthen and prolong blockade from non-depolarizing relaxants such as pancuronium, with risk of prolonged paralysis and respiratory depression. Oral neomycin carries this risk mainly when it is absorbed.
What this interaction means in plain words
Neomycin and Pancuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Neomycin belongs to Anti Bacterial; Pancuronium belongs to Musculoskeletal.
Why it happens
Aminoglycoside inhibition of presynaptic acetylcholine release and reduced postsynaptic sensitivity add to non-depolarizing blockade.
What to do
Monitor neuromuscular function with train-of-four and have ventilatory support ready through recovery. Reduce the pancuronium dose as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
