Summary
Palonosetron has rarely been linked to serotonin syndrome when combined with serotonergic drugs such as triptans. The risk is low and largely theoretical. Netupitant's CYP3A4 inhibition has little effect on rizatriptan, which is cleared by MAO-A.
What this interaction means in plain words
Netupitant / palonosetron and Rizatriptan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Possible additive serotonergic effect; the mechanism of 5-HT3 antagonist involvement is not well established.
What to do
Monitor for serotonin syndrome features such as agitation, tremor, sweating and clonus. Stop the agents and treat if symptoms arise.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
