Summary
Combining pentazocine with nortriptyline can increase drowsiness and psychomotor impairment. Pentazocine has weak serotonergic activity, and together with a tricyclic antidepressant there is a possible, though uncommon, risk of serotonin toxicity. Seizure threshold may also be lowered.
What this interaction means in plain words
Nortriptyline and Pentazocine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Nortriptyline belongs to Central Nervous System; Pentazocine belongs to Musculoskeletal.
Why it happens
Additive CNS depressant effects, with possible additive serotonergic activity and anticholinergic effects from the tricyclic.
What to do
Start with the lowest effective doses and warn the patient about sedation and impaired driving. Monitor for agitation, tremor, hyperthermia or clonus, and stop one agent if serotonin toxicity is suspected.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
