Summary
Nortriptyline has anticholinergic activity and can prolong QT/QRS, so combination with solifenacin increases antimuscarinic burden and may add to QT risk. This can cause urinary retention, constipation, confusion and arrhythmia, especially in older adults.
What this interaction means in plain words
Nortriptyline and Solifenacin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Nortriptyline belongs to Central Nervous System; Solifenacin belongs to Genitourinary.
Why it happens
Additive antimuscarinic effects and additive cardiac conduction/QT effects.
What to do
Monitor for anticholinergic toxicity and urinary retention, and obtain an ECG in patients with cardiac disease. Consider a non-anticholinergic bladder agent if adverse effects occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
