Summary

Opioids can precipitate serotonin syndrome or, with some opioids, severe reactions when combined with MAOIs. Tranylcypromine may also raise oliceridine exposure through CYP2D6 inhibition, increasing the risk of respiratory depression.

What this interaction means in plain words

Oliceridine and Tranylcypromine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Oliceridine belongs to Musculoskeletal; Tranylcypromine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Serotonergic excess plus CYP2D6 inhibition, and additive opioid CNS depression.

What to do

Avoid concurrent use where possible, including within 14 days of stopping an MAOI. If it is unavoidable, use reduced doses and monitor for respiratory depression, sedation and serotonin toxicity.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
