Summary
Both drugs lower blood pressure, and the effect may be additive. Olmesartan can reduce renal function and raise potassium, and reduced sotalol clearance and electrolyte disturbance can increase the risk of torsades.
What this interaction means in plain words
Olmesartan and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Olmesartan belongs to Cardiovascular; Sotalol belongs to Cardiovascular.
Why it happens
Additive antihypertensive effect; angiotensin receptor blockade may reduce glomerular filtration and cause hyperkalemia, altering sotalol clearance and cardiac repolarization.
What to do
Monitor blood pressure, heart rate, serum potassium, creatinine and QTc, especially at initiation and after dose changes. Adjust the sotalol dose according to creatinine clearance.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
