Summary
Ranolazine prolongs the QT interval in a dose-related manner, and beta2 agonists such as olodaterol can cause hypokalemia, tachycardia and some QT prolongation. Used together, they may add to the risk of ventricular arrhythmias, particularly in patients with cardiac disease or electrolyte disturbances.
What this interaction means in plain words
Olodaterol inhaled and Ranolazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive pharmacodynamic effects on cardiac repolarization and heart rate (ranolazine late sodium current and IKr inhibition; beta2-agonist adrenergic stimulation and hypokalemia).
What to do
Monitor ECG and serum potassium in patients with additional QT risk factors or structural heart disease. Correct hypokalemia and hypomagnesemia and report palpitations or syncope.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
