Summary
Botulinum toxin weakens muscles by blocking acetylcholine release, so neuromuscular blockers given near the time of injection may add to this weakness. Excessive generalized or local muscle weakness, including effects on swallowing or breathing, is possible.
What this interaction means in plain words
OnabotulinumtoxinA and Rocuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
OnabotulinumtoxinA belongs to Musculoskeletal; Rocuronium belongs to Musculoskeletal.
Why it happens
Additive neuromuscular blockade: presynaptic inhibition of acetylcholine release by the toxin plus postsynaptic nicotinic receptor antagonism by rocuronium.
What to do
Monitor neuromuscular function closely, with train-of-four monitoring perioperatively, in patients recently treated with botulinum toxin. Reduce the rocuronium dose as guided by monitoring and be prepared to provide ventilatory support.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
