Summary
Aminoglycosides impair neuromuscular transmission and can potentiate the effect of botulinum toxin. This may lead to excessive muscle weakness, including swallowing and breathing difficulty, and may be delayed after injection.
What this interaction means in plain words
Gentamicin and OnabotulinumtoxinA can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Gentamicin belongs to Anti Bacterial; OnabotulinumtoxinA belongs to Musculoskeletal.
Why it happens
Additive inhibition of neuromuscular transmission, with aminoglycosides reducing presynaptic acetylcholine release.
What to do
Avoid aminoglycosides around the time of botulinum toxin treatment when alternatives exist. If both are needed, monitor closely for generalized weakness, dysphagia and respiratory compromise.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
