Drug interactions
opioid + tramadol
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive opioid effects may cause profound sedation and respiratory depression; overdose risk. Tramadol also lowers seizure threshold. Serotonin syndrome possible if the concomitant opioid is serotonergic (e.g., meperidine, methadone, fentanyl).
Mechanism of action
Pharmacodynamic additive mu-opioid agonism → CNS/respiratory depression; tramadol’s serotonergic activity may interact with serotonergic opioids → serotonin toxicity; tramadol reduces seizure threshold.
Clinical management
Avoid combination when possible. If necessary, use lowest effective doses for the shortest duration; avoid concurrent initiation or rapid titration; monitor respiratory rate, O2 saturation, and sedation; ensure naloxone availability; monitor for serotonin syndrome if applicable.
