Summary
Both drugs can prolong the QT interval, so giving them together may produce additive prolongation and raise the risk of torsades de pointes. The risk is greater with electrolyte disturbances such as hypokalemia or hypomagnesemia, which are common during chemotherapy.
What this interaction means in plain words
Oxaliplatin and Selpercatinib can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Oxaliplatin belongs to Oncology; Selpercatinib belongs to Oncology.
Why it happens
Additive pharmacodynamic QT prolongation through effects on cardiac repolarization.
What to do
Obtain a baseline ECG and correct potassium and magnesium before starting. Repeat ECGs during treatment and consider an alternative agent if QTc becomes markedly prolonged.
What to ask your pharmacist
